Holiday Closure Notice

Please be advised that our surgery will be closed on the following dates: 15th May (from 1.00 pm for training) and on 26th May (Bank Holiday). If you require urgent medical assistance during these times, please contact the Out of Hours Service by dialling 111. The 111 service is available 24/7 and can provide advice, arrange appointments with an out-of-hours GP, or direct you to the nearest appropriate healthcare service based on your needs.

Subject Access Requests (SAR)

Requests for Accessing a Patient’s own Medical Record

The UK General Data Protection Regulation (UK GDPR) gives individuals the right of access to their personal data from any person or organisation that holds information about them. This right is commonly exercised through a ‘subject access request’ (SAR).

Patients are entitled to request information we hold about them. The easiest solution is to provide patients with online access to their GP record which will empower them to access their record themselves. If, however, copies of paper records are required, this can be organised, but it is a lengthier and more elaborate process. Please fill-in the request form below:

Personal information collected from you by this form, is required to enable your request to be processed, this personal information will only be used in connection with the processing of this Subject Access Request. By completing this form, you are making a request under the General Data Protection Regulation (GDPR) for information held about you by the practice that you are eligible to receive.  All requests are subject to approval by the GP.

Access to online medical records for children under 11 should be requested using the Proxy Access Form.
