Requests for Accessing a Patient’s own Medical Record
The UK General Data Protection Regulation (UK GDPR) gives individuals the right of access to their personal data from any person or organisation that holds information about them. This right is commonly exercised through a ‘subject access request’ (SAR).
Patients are entitled to request information we hold about them. The easiest solution is to provide patients with online access to their GP record which will empower them to access their record themselves. If, however, copies of paper records are required, this can be organised, but it is a lengthier and more elaborate process. Please fill-in the request form below:

Personal information collected from you by this form, is required to enable your request to be processed, this personal information will only be used in connection with the processing of this Subject Access Request. By completing this form, you are making a request under the General Data Protection Regulation (GDPR) for information held about you by the practice that you are eligible to receive. All requests are subject to approval by the GP.
Access to online medical records for children under 11 should be requested using the Proxy Access Form.
