Register with us as a New Patient

When you attend the Practice to register you will be asked to complete a GP Registration Form and New Patient Questionnaire Form. You will also be asked to provide two forms of Identification (ID); one photographic ID and one Proof of Address, or a birth certificate if registering a child.

If you are registering a child please ensure you also complete the Child Immunisation History form below.

Once we have received your form and ID, you will be registered on our system within one week and you will receive a confirmation letter in the post.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

Please note that your medical records may take several weeks to arrive at the Practice.

Please also ensure to send over your two forms of ID to our email address as well once you have completed your form, as won’t be able to proceed with your application without it. Our email address is [email protected].

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
